Application of somatosensory movement combined with percutaneous acupoint electrical stimulation rehabilitation nursing program in patients with thyroid cancer after operation:a randomized controlled trial

Objective To analyze the effect of somatosensory movement combined with percutaneous acupoint electrical stimulation rehabilitation nursing program on rehabilitation training after neck lymph node dissection for thyroid cancer. Methods The convenient sampling method was used to select 90 patients with thyroid cancer who were admitted to the head and neck surgery department of a tertiary A hospital in Zhejiang Province from May 2021 to January 2022 after neck lymph node dissection. According to the random number table method,they were divided into a test group and a control group,with 45 patients in each group. The test group implemented so-matosensory movement combined with percutaneous acupoint electrical stimulation rehabilitation nursing program,and patients in the control group received routine rehabilitation nursing training,with a total duration of 12 weeks. The differences of the neck range of motion,shoulder joint function assessment and cancer patients’ quality of life between the 2 groups before the intervention and at the 1st,4th and 12th weeks after the intervention were com-pared. Results Before the intervention,there was no significant difference between the 2 groups in the scores of the functional dimension and symptom dimension in the cervical range of motion,shoulder function rating scale and the quality of life scale for cancer patients(P>0.05). At the 1st,4th and 12th weeks after the intervention,there was an interaction effect between the 2 groups in the scoring time of the functional dimension and symptom di-mension in the cervical range of motion,shoulder function rating scale and the quality of life scale for cancer pa-tients(P<0.05). Conclusion Somatosensory exercise combined with percutaneous acupoint electrical stimulation re-habilitation nursing program can improve the neck mobility and neck shoulder joint function of patients with thy-roid cancer after surgery,and improve the quality of life.

Construction and application of a perioperative management program of breast reconstruction with deep inferior epigastric perforator flap in patients with breast cancer

Objective To construct a perioperative management program for breast reconstruction with deep inferior epigastric perforator flap with breast cancer,and to provide references for clinical practice. Methods The databases at home and abroad were searched to obtain the relevant literature,and the perioperative management program of breast reconstruction with inferior epigastric artery perforator flap for breast cancer was preliminarily constructed. 3 rounds of expert inquiries were conducted by Delphi method from October 2022 to January 2023 to determine the final content of the program. 14 patients who received breast reconstruction with deep inferior epigastric perforator flap in a tertiary hospital in Zhejiang Province from January to May in 2023 were selected as application subjects to implement the modified program and evaluate the clinical effect. Results A total of 15 experts completed 3 rounds of consultation. In the third round of consultation,the Kendall’s concordance coefficient of the importance of the first-level indicators was 0.347(P<0.05);the Kendall’s concordance coefficients of the importance of the second and third-level indicators were 0.388 and 0.200(P<0.001);the coefficient of variation of each indicator was 0~0.224. The final program included 3 first-level indicators,18 second-level indicators and 67 third-level indicators. The incidence of flap complications in 14 patients decreased by 11.9%. Conclusion The perioperative management program of breast reconstruction with deep inferior epigastric perforator flap for breast cancer is preliminarily applied in clinical practice,which is scientific,reliable and practical,and it can provide references for clinical nursing.

Evidence summary of thirst management in patients with oral tracheal intubation in ICU

Objective To extract,evaluate and summarize the relevant evidence of thirst management of patients with oral tracheal intubation in ICU,and to provide evidence bases for medical staff for scientific management. Methods The BMJ Best Practice,CINAHL,Cochrane Library,Clinical Trials,Embase,PubMed,UpToDate,Web of Science,Guidelines International Network(GIN),National Guideline Clearinghouse(NGC),National Institute for Health and Clinical Excellence(NICE),Joanna Briggs Institute(JBI),Registered Nurses’ Association of Ontario(RNAO),Scottish Intercollegiate Guidelines Network(SIGN),CNKI,SinoMed,Wan Fang database,VIP database and Yimaitong Guidelines Network were systematically searched for clinical decisions,evidence summaries,guidelines,recommended practices,expert consensuses,systematic reviews,and randomized controlled trials and the retrieval time limit is from the establishment of the database to November 20,2022. 2 researchers who were trained in evidence-based nursing independently evaluated the quality of the literature,and extracted and summarized the evidence in combination with expert suggestions. Results A total of 17 articles were included,including 4 clinical decisions,1 evidence summary,4 systematic reviews,and 8 randomized controlled trials. 26 pieces of best evidence were summarized on the management of thirst in ICU patients undergoing oral tracheal intubation,including 4 aspects:identification of risk factors for thirst,evaluation,intervention,and evaluation of the effectiveness of intervention measures. Conclusion The best evidence summarized in this study can provide evidence-based bases for clinical medical staff to manage the thirst symptoms of patients with oral tracheal intubation in ICU. When applying evidence,medical staff should correctly use the evidence in combination with the patient’s wishes and specific clinical situations to solve the patient’s thirst problem.

Analysis on the status quo and influencing factors of clinical nurses’ disaster preparedness ability

Objective The current situation of disaster preparedness ability of clinical nurses in general hospitals was investigated and analyzed,and the possible factors affecting the disaster preparedness ability of nurses were discussed,so as to provide theoretical basis for carrying out targeted training to improve the disaster preparedness ability of nurses and guiding the dispatch of disaster rescue nurses. Methods From August to September 2022,with convenience sampling method and general information questionnaire and Readiness Estimate and Deploy Ability Index,clinical nurses from 2 to 3 tertiary general hospitals in each region were selected to conduct questionnaire survey. Results A total of 1 048 valid questionnaires were collected,and the total score of clinical nurses’ disaster preparedness ability was (118.71±29.17) scores,and the average score of each item was(3.21±0.79) scores. The scores of the items of identity were generally higher than those of the items of disaster rescue ability. Among the 6 dimensions of the scale,disaster care ability scored the lowest,with only(2.64±1.04) points. Nurses’ working years,educational background,batch treatment experience,confidence in completing diversified tasks and the unit’s normal first-aid training mechanism were the possible factors influencing clinical nurses’ disaster preparedness ability(P<0.05). Conclusion In this study,nurses’ disaster preparedness ability was at a medium level,and the scores of various dimensions and items of disaster preparedness ability were significantly different. Affected by various factors,nursing managers should attach importance to the training of nurses’ disaster preparedness ability,establish an evaluation,hierarchical training and assessment mechanism,and continuously improve nurses’ comprehensive ability to deal with emergencies.

Effect of health education based on information-motivation-behavior model on paternal self-efficacy of support breastfeeding of late prematurity

Objective To explore the effect of information-motivation-behavioral(IMB) model on self-efficacy of fathers of late preterm infants in support of breastfeeding. Methods A total of 86 fathers of late preterm infants who were transferred to the neonatology department immediately after delivery in a tertiary A hospital in Taiyuan City from October 2021 to February 2022 were conveniently selected as the research objects. According to the length of hospitalization,43 fathers from December 2021 to February 2022 were selected as the experimental group. 43 fathers from October to November 2021 were used as a control group. The experimental group received IMB model based on routine nursing,and the control group received routine nursing. The scores of self-efficacy of paternal breastfeeding support and exclusive breastfeeding rate were compared between the 2 groups at different time points. The scores of breastfeeding knowledge,behavior and attitude of the 2 groups were compared,as well as the incidence of insufficient lactation. Results The self-efficacy scores of fathers supporting breastfeeding in the experimental group were higher than those in the control group at discharge and 1 month after discharge,and the difference was statistically significant(P<0.001). With the extension of time,the self-efficacy scores of each group increased gradually. The scores of fathers’ breastfeeding knowledge and behavior at discharge in the experimental group were higher than those in the control group;the incidence of maternal insufficient lactation in the experimental group was lower than that in the control group;the exclusive breastfeeding rate of preterm infants at 40 weeks,3 months and 6 months of adjusted gestational age was higher than that in the control group;the difference was statistically significant(P<0.05). Conclusion Nursing intervention based on IMB model can effectively improve the self-efficacy and scores of breastfeeding knowledge,behavior and attitude of fathers of late preterm infants to support breastfeeding,reduce the pressure of fathers to take care of mothers and preterm infants,so as to increase the rate of exclusive breastfeeding of premature infants.

Objective To construct a training program of sexual health care for patients with cervical cancer for gynecological oncology nurses,and provide references for the theoretical research and clinical practice of improving the sexual health care ability of gynecological oncology nurses for patients with cervical cancer. Methods Based on literature review and qualitative interview,the first draft of a letter inquiring about the training program of sexual health care on cervical cancer patients for gynecological oncology nurses was formulated. From September to December 2020,15 gynecological oncology medical experts,gynecological oncology nursing experts and sexual science experts from 4 provinces and cities of Beijing,Tianjin,Shanghai and Jilin Province were interviewed by Delphi experts for 2 rounds to formulate the training program of sexual health care on cervical cancer patients for gynecological oncology nurses. Results The effective recovery rates of 2 rounds of expert consultations were 93.75% and 100%. The authority coefficients of the authority coefficients were 0.843 and 0.853.The Kendall’ W was 0.429 and 0.326. The coefficient of variation of items in the second round was 0~0.115,and the full score rate is 20%~100%.The constructed training program of sexual health care on cervical cancer patients for gynecological oncology nurses consists of 3 parts,among which the training content includes 6 first-level indicators,16 second-level indicators,48 third-level indicators,and 6 training methods and 6 assessment methods. Conclusion The process of constructing training program of sexual health care on cervical cancer patients for gynecological oncology nurses is authentic and reliable,with distinctive specialty features,various training methods and assessment methods,which can meet the needs of sexual health care training for gynecological oncology nurses,and can promote the scientific and standardized development of sexual health care.

Effect of external diaphragm pacer on prevention of ventilator-induced diaphragmatic dysfunction in patients with ICU non-invasive ventilation

Objective To establish the rehabilitation program of external diaphragm pacer and to explore its preventive effect on ventilator-induced diaphragmatic dysfunction(VIDD) in patients with ICU non-invasive mechanical ventilation. Methods Through the literature review,group meeting and pre-experiment,a program was formulated to prevent VIDD in patients with non-invasive ventilation in ICU. Patients with non-invasive ventilation were conveniently selected from September 2020 to June 2021 in a respiratory intensive care unit of a tertiary hospital in Henan province. They were divided into 2 groups by random number table method. On the basis of routine nursing care,the experimental group was given the external diaphragm pacer training program,and the control group was given routine nursing program. The incidence of VIDD,diaphragm thickness score,diaphragm displacement and blood gas analysis were compared between 2 groups after the intervention. Results A total of 74 patients were included with 10 cases fallen off,and finally 64 cases were included with 32 cases in the experimental group and 32 cases in the control group. After the intervention,the incidence of VIDD was 6.25% in the experimental group,while that was 43.75% in the control group. The difference between 2 groups was statistically significant(P=0.001). Conclusion The external diaphragm pacer rehabilitation program can reduce the incidence of VIDD and improve diaphragm and respiratory function in patients with ICU non-invasive mechanical ventilation.

Best evidence summary for sexual health management in breast cancer patients

Objective To search,evaluate and integrate the relevant evidence for sexual health management in breast cancer patients,and to provide references for clinical practice. Methods We searched CNKI,CBM,UpToDate,PubMed,Cochrane Library,Joanna Briggs Institute,CINAHL,National Guideline Clearinghouse,National Comprehensive Cancer Network,American Society of Clinical Oncology,European Society for Medical Oncology for relevant clinical decisions,guidelines,systematic reviews,evidence summaries and expert consensuses. The retrieval period is from the establishment of databases to February 10,2022. Results A total of 17 articles were enrolled,including 4 clinical decisions,5 guidelines,4 expert consensuses and 4 systematic reviews. Totally 33 pieces of best evidence were summarized from 7 aspects,namely screening and evaluation,psychological intervention,instrumental and exercise intervention,non-hormone therapy,hormone therapy,related reproduction and training and follow-up. Conclusion This study summarized the best evidence for sexual health management in breast cancer patients. In clinical practice,nurses can personalize the application of evidence according to clinical situation and patients’ wishes to improve breast cancer patients’ sexual health.

A multicenter survey on the status and demand of humanistic nursing training for nurses

Objective To understand the status and training needs of humanistic nursing training for nurses,so as to provide bases for the development of humanistic nursing training programs and targeted humanistic nursing in-service training for nurses. Methods From August to September 2021,2 3707 nurses from 28 provinces were investigated by convenient sampling. A self-designed questionnaire was used and launched through the platform of Wenjuanxing. Results 99.71% of nurses had participated in at least a humanistic nursing training;54.61% of nurses had 26 training experiences;the training rate and demand rate of 26 humanistic nursing training contents were 74.63%~98.30% and 88.75%~96.35%,respectively. The main ways for nurses to participate in training were pre-job training(77.08%),stratified core competence training(73.56%),and hospital public training(72.92%). The training forms mainly included theoretical teaching(85.96%),video learning(72.61%),and case teaching(64.17%). The majority of trained teachers were nursing managers (89.19%). Single training form (49.42%),lack of targeted content(46.78%),unsystematic training courses(37.47%),weak training teachers(36.87%) are the main problems existing in the current training. 95.02% of the nurses thought that it was necessary to establish systematic and standardized humanistic nursing practice ability training courses for clinical nurses;87.93% of the nurses thought that it was necessary to carry out humanistic nursing specialized nurse training;46.19% of the nurses chose the training mode of theory and practice. 93.54% of nurses hope to receive theoretical knowledge training online. Conclusion Humanistic nursing in-service training of nurses in China has a wide range of training content,high training rate,and great training demand. The training courses,training forms,and training teachers should be further advanced. It is urgent to standardize the humanistic nursing in-service training program and construct a humanistic nursing training mode to improve the training effect.

Freezing of gait(FOG) is common in patients with Parkinson’s disease(PD). The incidence of FOG in PD patients in China is close to 50%. In order to effectively respond to and control FOG,it is important to choose scientific and effective assessment tools for objective and accurate evaluation. The application status of the common assessment tools for FOG in patients with PD at present is introduced in this review,in order to explore the advantages and disadvantages of these tools,so as to provide the basis for the selection of the best evaluation tools for patients.

Development and evaluation of four-stage early rehabilitation program for patients with acute heart failure in compensatory period

Objective To develop a four-stage early rehabilitation program in patients with acute heart failure in compensatory period and evaluate its effects,so as to provide references for their early rehabilitation activities. Methods A research team of early rehabilitation program was established,and a four-stage early rehabilitation program for patients with acute heart failure was initially formulated after literature retrieval and semi-structured interviews. The final plan was formed after the expert panel evaluation. A total of 200 patients with acute heart failure admitted to CCU from April 2017 to June 2019 were recruited by convenient sampling method with 100 patients in the control group and 100 patients in the experimental group. The control group received the regular activity plan,while the experimental group was provided with the four-stage early rehabilitation program. The Minnesota Quality of Life Scale,the 6 min walking test,the average length of hospitalization,the forced expiratory volume in one second (FEV l),and the brain natriuretic peptide(BNP) were adopted as monitoring indicators. Results The Minnesota quality of life in the experimental group scored(52.64±5.60) points,which was lower than(56.89±6.59) points( t=-4.877, P< 0.001) in the control group. The distance of 6 min walking test group was(279.86±84.66) m and it was better than(248.78±93.73)m( t=2.441, P=0.016) in the control group. The average length of CCU hospitalization in the experimental group was(8.97±2.80) d,which was less than(10.48±3.80) d( t=-3.182, P=0.002) in the control group. The FEV l in the experimental group was(2.18±0.49) L,which was higher than(1.92±0.35) L( t=4.338, P< 0.001) in the control group. The BNP level in the experimental group was(697.88±135.58) pg/ml which was smaller than(755.29±141.74) pg/ml( t=-2.905, P=0.004) in the control group. Conclusion The four-stage early rehabilitation program can formulate an individualized activity strategy based on patients' heart function,and guarantee the safety of early activities through team management methods,which can effectively improve the quality of life of patients with acute heart failure,promote the rapid rehabilitation of cardiopulmonary function,and shorten hospitalization time.

Objective To explore the effects of multidisciplinary teamwork intervention for total knee arthroplasty(TKA) patients with kinesiophobia. Methods A total of 106 TKA patients with kinesiophobia admitted from February 2018 to March 2019 in our hospital were enrolled as research subjects by a convenience sampling method.53 patients admitted from October 2018 to March 2019 were allocated into an experimental group, while 53 patients admitted from February to August 2018 were assigned into a control group. Multidisciplinary teamwork intervention was adopted in the experimental group with routine nursing care, while the control group only received routine nursing care. The differences of the 2 groups in the scores of kinesiophobia, knee function, pain severity in resting and exercising, the first activity time after getting out of bed, length of stay and hospitalization expenses were compared on discharge day. Results After the implementation of multidisciplinary teamwork intervention, the scores of kinesiophobia, pain severity in resting and exercising in the experimental group were lower than those in the control group( P< 0.05), and the score of knee function was higher than that in the control group( P< 0.05). In addition, the first activity time after getting out of bed, length of stay and hospitalization expenses in the experimental group were also better than those in the control group( P< 0.05). Conclusion The multidisciplinary teamwork intervention was scientific and reliable in TKA patients with kinesiophobia, which can reduce the incidence of kinesiophobia, relieve pain and accelerate the recovery of knee function effectively.

Development and psychometric testing of a Gout Patient Self-management Assessment Scale

Objective To develop a Gout Patient Self-management Scale and to test its reliability and validity. Methods Based on self-management theory,literature review and semi-structured interview,the provisional Gout Patient Self-management Assessment Scale was formed by Delphi technique and pilot investigation. Totally 260 gout patients were recruited to perform the item analysis for the provisional scale. Other 317 gout patients were investigated to test the reliability and validity of the final scale. Results The Gout Patient Self-management Assessment Scale consisted of 41 items in 4 dimensions. The Cronbach’s α coefficient of the total scale was 0.962;split-half reliability coefficient was 0.842;2-week test-retest reliability was 0.904. 4 common factors were extracted by exploratory analysis and the cumulative variance contribution rate was 60.779%. The content validity index(S-CVI) of the total scale was 0.905. Conclusion The Gout Patient Self-management Assessment Scale is a reliable and valid scale for measuring the self-management ability of gout patients.

Translation,revision and assessment of reliability and validity of the Quality of Oncology Nursing Care Scale

Objective To translate and revise the Quality of Oncology Nursing Care Scale(QONCS) into Chinese,then to assess the reliability and validity of the Chinese version of the Quality of Oncology Nursing Care Scale(C-QONCS).Methods Standard forward-back translation techniques were used in the translation of the QONCS according to the Brislin translation model. Cross-cultural revision of the translated QONCS was carried out through group discussion and pilot test. Totally 612 patients with advanced cancer were recruited and investigated using general information questionnaire and the C-QONCS by convenient sampling,and its reliability and validity were assessed.Results The C-QONCS had 5 dimensions:being supported and confirmed(16 items),spiritual caring(6 items),sense of belonging(5 items),being valued(4 items),being respected(3 items). The Cronbach’s a coefficient for the overall scale was 0.946,and ranging from 0.735 to 0.928 for each domain. Split-half reliability was 0.879 for the overall scale,and 0.683~0.864 for each domain. Test-retest reliability was 0.867 for the overall scale,and 0.837~0.889 for each domain. Analysis of content validity of the C-QONCS indicated that I-CVl was at least 0.86,S-CVl was 0.97.Conclusion The reliability and validity of C-QONCS are satisfactory and well meet the requirements of psychological measurement,indicating C-QONCS is a reliable and valid instrument in the context of Chinese culture,and can be used to assess the quality of oncology nursing care.

Objective To construct a nursing education knowledge system on dementia suitable for geriatric practicing nurses,and to provide reference for developing targeted training programs.Methods Based on literature research,semi-structured interviews and group discussion,the first draft of inquiry questionnaire was established. Delphi method was used to conduct two rounds of consultation with 21 experts in dementia-related fields.Results Response rate of two rounds of consultation was 100%, respectively. Experts’ authority coefficient was 0.900,judgment coefficient and familiarity coefficient was 0.820,and the coefficients of concordance were 0.425,0.256 for two levels of indicator. The final knowledge system consisted of six first-level indicators, and 81 second-level indicators.Conclusion The nursing education knowledge system on dementia for geriatric practicing nurses was scientific and practical, which can provide reference for making training programs on dementia.
